Indiana Life Celebration Quilts commemorate the lives of those who have given the gift of life through organ and tissue donation. Families of donor heroes create these unique quilts by designing quilt squares in honor of their loved ones. Once assembled, the quilts are displayed at events across the state to recognize donors and increase awareness of organ and tissue donation.

Families of donor heroes may find that participating in the Indiana Life Celebration Quilt not only furthers their loved one’s legacy, but also inspires hope and healing. Each family may create up to two patches for each donor. Family members are encouraged to work together in designing the patch, making it a memorable activity for all.

There is no timeline for submitting quilt patches. Patches, along with the form below, can be mailed to Indiana Donor Network at 3760 Guion Rd. Indianapolis, IN 46222. Please email aftercare@INDonorNetwork.org with questions or for more information.
